Which heart test has your doctor asked for?
People often arrive with a prescription and no idea what the letters mean, or why one test was chosen over another. Here is the short version.

These tests are complements, not alternatives. It is completely normal for a doctor to ask for an ECG and a 2D Echo together, or a TMT after a normal resting ECG. None of them is a substitute for seeing your doctor — we perform and report the test; the treatment decision is theirs.

How to prepare
Most cardiac tests need very little preparation. The treadmill test is the exception, and getting it wrong usually means coming back another day.
- ECGNo preparation at all. You lie down, electrodes are placed on the chest, arms and legs, and the recording takes under a minute. Avoid body lotion or oil on the chest, since electrodes stick poorly to it.
- 2D EchoNo fasting, no preparation. You lie on your left side while the probe is moved across the chest with gel. Men may be asked to remove the shirt; a gown is provided. Allow about 20 to 30 minutes.
- TMT — the one that needs planningCome with a light meal roughly two hours beforehand, not on a completely empty stomach and not straight after a heavy meal. Wear loose clothes and proper walking shoes — not sandals or slippers, which are the most common reason a test has to be rescheduled. Avoid tea, coffee and smoking for a few hours before.
- Your medicines, and the TMTSome heart medicines — beta blockers in particular — blunt the rise in heart rate and can make a treadmill test uninterpretable. Your doctor may ask you to withhold a dose. Do not stop any medicine on your own. Bring your prescription and tell us what you take when you book.
- HolterBathe before you come, because the recorder and electrodes stay on and must not get wet. Wear a loose top. You then go about your normal day and keep a short diary of any palpitation, giddiness or chest discomfort and the time it happened — that diary is what makes the recording useful.
- Bring with youYour prescription, previous ECG and echo reports, and a list of your current medicines. A test read against your previous ones is far more informative than one read on its own.
When to stop waiting and see a doctor
A diagnostic centre should say this plainly. Booking a test is the right move for symptoms you have had for a while. It is the wrong move for symptoms happening now.
Do not book a test — go to a hospital emergency department immediately if you have chest pain or pressure lasting more than a few minutes, chest discomfort spreading to the arm, jaw, neck or back, sudden severe breathlessness, fainting, or a fast irregular heartbeat with sweating or nausea. These need a doctor within minutes, not an appointment.
For breathlessness on climbing stairs, ankle swelling, palpitations that come and go, giddiness, a murmur your doctor has heard, a family history of early heart disease, or a pre-employment or insurance requirement — a scheduled test is exactly right, and you can walk in.

What is the difference between a 2D Echo and a TMT?
They answer different questions. A 2D Echo is an ultrasound that shows how the heart is built and how well it pumps — chambers, valves, wall movement and ejection fraction — while you lie still. A TMT watches the heart’s electrical behaviour while you walk on a treadmill under rising effort, which is how narrowing that only shows up under exertion is picked up. Doctors frequently ask for both.
Do I need to fast before an ECG or 2D Echo?
No. Neither test needs fasting or any other preparation. Only the TMT has real preparation: a light meal about two hours before, walking shoes rather than sandals, and no tea, coffee or smoking for a few hours beforehand.
How long does each test take, and when will I get the report?
An ECG takes a few minutes and the report is available the same day. A 2D Echo takes about 20 to 30 minutes. A TMT takes roughly 30 to 45 minutes including preparation and the recovery period afterwards. Holter monitoring runs for 24 hours or longer, and the report follows once the recording is downloaded and analysed.
Should I stop my heart medicines before a TMT?
Only if your own doctor tells you to. Some medicines, especially beta blockers, hold the heart rate down and can make a treadmill test impossible to interpret, so a doctor sometimes asks for a dose to be withheld. Never stop a prescribed medicine on your own decision. Bring your prescription and tell us what you are taking when you book, and we will flag anything that matters.
Is a TMT safe? Can anyone have one?
A supervised treadmill test is a routine investigation, but it is a stress test by design, so it is not for everyone. It is not done during an active heart attack, unstable chest pain, uncontrolled rhythm problems, severe aortic valve narrowing or uncontrolled high blood pressure, and it needs care in anyone who cannot walk safely. You are monitored throughout and the test is stopped the moment anything looks wrong. Your referring doctor decides whether a TMT is appropriate for you.
Do I need an appointment, or can I walk in?
An ECG can be done on a walk-in basis. For a 2D Echo, a TMT or Holter monitoring it is better to fix a slot, because these need a specific room and staff time. Send your prescription on WhatsApp and we will hold the earliest slot for you.
Do you need a doctor’s prescription for a heart test?
Bring one if you have it. A prescription tells the reporting doctor what question the test is meant to answer, which makes the report far more useful to whoever treats you. It also lets us check whether the test your doctor wants is the one you are booking.
